Patient having chipped a front tooth
1a

Same patient after restoring chipped tooth. Note a very old white filling awaiting replacement in the neighbouring tooth. If set properly and polished up well. Modern white fillings should not discolour like they did in the past

This Patient complains of a small discoloured area on her front tooth, as well as slight chipping of the edges
9a
After careful air abrasion to remove the weak enamel it is restored in composite. Note the slight change in shape of the edges, this was done by sympathetic polishing only

Following the extraction of a front tooth the socket was not augmented and the bone has shrunk making it difficult to provide a bridge or implant without some further work

This patient had two teeth out at the bottom leaving spaces many years ago. It allowed her bottom teeth to slide forwards over closing and wearing away
2a
One of the gaps at the bottom
2b
By building up the biting surfaces of the back teeth in composite we could make more room at the front to allow the bottom incisors to be veneered in composite. Note the upper canines have also been built back up along with the spaces at the bottom being bridged in composite

Although we do not have a photo before treatment showing this patient over closed (you could barely see the bottom teeth when the teeth were together) you can now clearly see the bottom teeth when clenched

Crowns and Posts

The problem of porcelain bonded to metal is that the neck of the tooth often looks black. This is due to several reasons i) The metal does not allow light to penetrate the crown and so it looks black ii) Metal ions leach out of the crown or post into the tooth actually discolouring it
1a
After building up the back teeth, the front teeth could then be placed forwards giving a much more youthful appearance
1b

There is no point making a non-metal crown if you are going to use a metal post
2a
Fibre posts not only allow light to pass through them but they ‘give’ a little under stress so the root is less likely to fracture which is a huge problem with metal posts
2b
Compare this fibre post and core to photo 2a
